Every inquiry gets a real answer from a person. If what you're asking about isn't supported by evidence, we'll say so.

The theory behind it

In theory, this class is designed to speed the middle stages of healing. Promoting new blood vessel growth into damaged tissue and mobilising cells into the wound bed, so that tendon, ligament and gut repair happens faster and more completely. The mechanisms are real and well described in preclinical models. The gap between a standardised laboratory wound and a human degenerative tendon is where the theory currently sits untested.

Unmarked, FDA approved° legal over the counter† under FDA review, not yet permitted* not FDA approved‡ documented safety concern

CompoundWhat is known
BPC-157†Advisory panel voted 8–6 in favor July 24, 2026. FDA's own scientists opposed listing, citing poor characterization. First human RCT began recruiting April 2026.
TB-500†Advisory panel voted favorably July 2026. Widely used on essentially no controlled human data.
KPV†Advisory panel voted favorably for wound healing and inflammatory conditions, on minimal human evidence.
GHK-Cu°Topical is a legal cosmetic ingredient with the best evidence in the category. Injectable is on a separate, unfinished track.
LL-37†Off Category 2. Advisory review before end of February 2027. Minimal human data.
ARA-290 (cibinetide)*Investigational for neuropathic pain. Not approved.
B7-33*Preclinical only.
BPC-157 + TB-500*A marketed combination. Neither component has controlled human efficacy data, so the pairing has none either.
GLOW / KLOW blends*Proprietary multi-peptide blends. Blends are not separately studied. Evidence for a mixture is not the sum of its parts.
Cortagen, Cartalax, Crystagen, Cardiogen*Russian bioregulator peptides. Essentially no peer-reviewed Western clinical data.

The science behind this category

The biology

Wound healing runs through overlapping phases: inflammation, new blood vessel formation, cell migration into the wound bed, matrix deposition, and remodelling. The peptides here target the middle stages, mostly by promoting angiogenesis or by mobilising cells into damaged tissue. The mechanisms are real and reasonably well described.

Why the preclinical data does not transfer

Preclinical healing models are the source of nearly everything claimed here, and they are systematically optimistic. These models heal faster than humans, the injuries are standardised and acute rather than chronic and degenerative, dosing is by body weight in ways that rarely scale, and outcomes are measured over days. Human tendinopathy is a different problem in a different tissue on a different timescale. This is not a criticism of the research. It is a limit on what the research can tell you.

What the July 2026 vote actually meant

BPC-157, TB-500, and KPV all received favourable advisory recommendations. It is worth being precise about what that means: the committee was not asked whether these compounds work. It was asked whether pharmacies should be permitted to compound them. Several members argued explicitly that regulated compounding is safer than the grey market people are already using, an access argument, not an efficacy one. FDA's own scientists recommended against, citing insufficient evidence.

The characterisation problem

FDA's more fundamental objection to BPC-157 was not safety but identity. Reviewers stated the substance is not well characterised, and that quality standards cannot be set until it is. That is an unusual objection, and it matters for anyone buying: if the agency cannot define what the substance is, a certificate of analysis has nothing firm to certify against.

Angiogenesis cuts both ways

The mechanism that makes these compounds interesting for healing. Promoting new blood vessel growth. Is also how tumours establish blood supply. There is no human data showing harm, but there is also none showing safety, and this is the specific reason caution is warranted in anyone with a cancer history.
